129 Medication Errors

Reports about the impact medication errors have on patients and their families as well as healthcare providers are frequently in the news.  Hospitals, government agencies, drug manufacturers, and The Joint Commission (TJC) are all trying to create strategies to decrease and even eradicate these costly mistakes. 

This two-part series focuses on the foundation of medication delivery to patients in all settings: the Eight Rights of Medication Administration.  In the first program, each of these rights is explained in detail and demonstrated using vignettes.  Throughout, the National Patient Safety Goals developed by TJC are highlighted.  The second program is a series of case studies.  Here, the learner is encouraged to use critical thinking skills to apply the information he or she has seen in the first video by recognizing the medication errors made as well as ways in which these mistakes could have been avoided.

1. List the components of a medication order.
2. Explain the process for taking a telephone order for medication.
3. State how orders are transcribed from the order sheet to the patient’s Medication Administration Record (MAR).
4. Describe in detail each of the Eight Rights of Medication Administration.
5. Outline The Joint Commission Patient Safety Goals that are concerned with medication administration.
6. Find appropriate medication administration references and resources.
7. Identify situations in which medication errors may occur and methods to prevent them.

1. Describe in detail each of the Eight Rights of Medication Administration.
2. State the components of a complete medication order.
3. Identify the medication errors made in the case studies presented.
4. Discuss methods in which the errors made in these scenarios could have been prevented.
